New Patients
At Belmoont Dental Care, we are pleased to be able to offer both NHS and Private dental services. We welcome new patients of all ages who can choose to register either as NHS patient, Private patient or an IndepenDent Care Plan patient. All patients wishing to register as a new patient can come to the practice to see our friendly Reception team and collect a Medical History Form. Alternatively, patients can register by phoning our Reception team on 020 3827 4527 or online via our website and a Medical History Form will be emailed to you.
Please ensure that your Medical History form (including details of all medications you may be taking) is completed before your dental appointment and we request that you arrive 10 minutes prior to your appointment time. The following documents must also be brought to your first appointment at the practice to complete the registration process:
Photo ID

Proof of address
Details of your Doctor / GP
NHS Number (obtained from your GP if registering as an NHS Patient)
If applicable, relevant proof of exemption from paying NHS charges (e.g. maternity exemption, universal credit)
